Before you complete your enrollment, we want to make sure a few key points are crystal clear. This helps avoid any confusion and ensures you know exactly what you’re signing up for!
What You’re Signing Up For
Membership Model: TrueCare DPC is a membership-based practice. You’ll pay a flat monthly fee for access, not for each individual visit or service.
No Insurance Billing: We do not bill insurance for any reason. All membership payments are direct between you and TrueCare DPC.
Monthly Fee: The membership fee is due every month, whether or not you are seen that month. Think of it like a gym membership—you’re paying for ongoing access.
Commitment Terms
Initial Commitment: Enrollment includes a four-month minimum membership.
Payment Schedule: You will not be asked to pay all four months up front—just one month at a time.
Early Cancellation: If you cancel before your four months are complete, you’ll be responsible for 50% of the remaining balance.
Important Details
Agreement: Please take time to read the Membership Agreement carefully at the end of your enrollment form.
Billing Info: When you complete your enrollment, you’ll be asked to provide billing information.
You will not be charged immediately.
Charges begin only after you’ve spoken with your chosen Primary Care Manager (PCM).
Choosing a PCM: As part of the form, you’ll select your PCM from a dropdown list. This ensures you know who will be caring for you.
Once you’ve read this, you can finish the form with confidence. After you click submit, your PCM will reach out to you before any charges occur.
